Resources Available from Community Health Workers Call and Person-Centered Care Quality Measures Webinar

In late January, Community Catalyst hosted a learning community call about the role of Community Health Workers in Health System Transformation. Materials are now available from the presentations shared on the call by Angela Jenkins, Project Manager of the Value Advocacy Project at Community Catalyst and Juan Pinzon, Harlem Health Advocacy Partners in New York.

Last month, Community Catalyst hosted a webinar on Patient Engagement and Person-Centered Care Quality Measures. Presenters included Emily Henke, Policy Manager at the Oregon Public Health Institute, who discussed the Consumer Confidence Project. This project is a grassroots evaluation of Oregon’s Coordinated Care Organizations, focusing on consumer-oriented measures. The presentation is available.

Two New Briefs on Dual Eligible Demonstration Projects

The Center for Health Care Strategies has released two briefs related to the dual eligible demonstration projects. The first brief discusses how seven states are communicating early results of their demonstrations, highlighting three dissemination strategies that could be replicated by other states. The second discusses how eight states are developing and refining Interdisciplinary Care Teams (ICTs), which are an essential component of integrating care for Medicare/Medicaid enrollees. It focuses on how prescriptive ICT requirements should be, how ICTs can better engage providers and how to measure ICT performance.

The National Academy for State Health Policy has released a map highlighting states with active Medicaid payments to health and medical homes.
